AdSense is the biggest online publisher program for website and blog owner to make huge money from their content. To apply for this program you have to eligible for their terms and conditions, otherwise you will not be approved for this money making program. Actually Google AdSense have strict policies for publishers to satisfy their Advertisers. - This form contains - Name- In this column you have to fill your name as you have filled out for payee name.
Publisher ID- in this column you have to fill out your publisher ID ( like - pub-12345xxxxxxxxxx )\
Issue ID- you can find Issue ID in the violation email that is received by Google
Affected site URL- URL of your banned website
What changes you made- in this column you have to explain that how you change your website and improved quality of your website. You have to tell them that you have improve your mistakes that resulted in your banned website.
- Now submit this form and wait until you will receive an email with reply for your appeal form.
But if you did't receive ant email in the form of reply, then you can contact them to their email adsense-support@google.com
